We have the Moderna vaccine back in stock and are offering it on a walk in basis Tuesday - Thursday from 10:00am - 4:00pm. We offer first, second & booster doses at Lakeside Pharmacy.

To save time, be prepared with the following items:

1. Complete a pre-vaccination questionnaire. We have copies at the pharmacy or you can download here and complete it before you come in:

2. Bring your prescription insurance card and your Medicare Part B card (if you have Medicare) with you when you come in for your vaccine

3. If you have had a COVID vaccine before, bring your vaccination record with you

4. Wear appropriate clothing - we will need access to the area just below your shoulder.
