Schedule COVID Vaccine & COVID Testing

Lakeside Pharmacy is now offering COVID Vaccine and Rapid COVID tests by appointment. Due to an increase in demand for vaccine and the fact that we are adding COVID testing to our services, we had to move to an appointment based system for both. Directions for scheduling are below. If you need assistance scheduling an appointment, please call the scheduling service at (425) 655-2300 and someone can help you.




To schedule a COVID vaccine (we have the Moderna vaccine - https://www.modernatx.com/covid19vaccine-eua/eua-fact-sheet-recipients.pdf ):

  1. Click the link for the vaccine you need (First, Second, Booster). ** updated 1/14/22 -You are eligible for a Moderna booster dose if you:

a. Received your first 2 doses of Pfizer or Moderna vaccine with the second

dose being 5 months or more ago

b. Received your dose of Johnson & Johnson vaccine 2 months or more ago.

  1. After clicking the link, you will be directed to the scheduling platform. Scroll down to the box labeled Clinical Services and click "Get Started".

  2. You will be asked to set up an account with your cell phone. You can help someone else sign up using your cell phone - you will be asked if the appointment is for you or someone else.

  3. Answer all of the questions.

  4. On the day of your appointment, remember to wear loose clothing so we can access your shoulder. Check in with an employee when you arrive at the pharmacy and be prepared to provide the following: Insurance card for billing purposes, Vaccine card to verify dates of previous doses, ID or proof that you live or work in Washington State.





To Schedule a COVID Test:

  1. Click the link for the test you need. Washington Department of Health provides free testing for people who: have symptoms of COVID-19, have been exposed -been within 6 feet for 15 minutes or more in a 24 hour period- to someone confirmed or suspected of having COVID-19, or for people who have been told by a healthcare provider or public health official to get tested. If you do not qualify for the free test, we will also be offering Rapid COVID tests for people who are needing that information for a small fee. At this time we are not offering the PCR test for travel purposes.

  2. After clicking the link, you will be redirected to the scheduling platform. Scroll down to the box labeled Clinical Services and click "Get Started".

  3. You will be asked to set up an account with your cell phone. You can also help someone else make an appointment using your cell phone - you will be asked if the appointment is for you or someone else.

  4. Answer all of the questions.

  5. On the day of your appointment please pull around to the back of the pharmacy and park next to the orange cone labeled "Testing". Call us to let us know you are here and someone will come out to help you. The testing will be done inside your vehicle, please DO NOT come in to the pharmacy.
